Please use this form to request a repeat prescription for your pet’s medication. We will let you know when it’s ready for collection so that you can drop in to complete payment and collect at a time that suits your schedule.

Our ability to fulfil this request is dependent on your pet’s current medical condition and the time elapsed since your last visit with us. We need your pet to have been assessed within the last 6 months by one of our veterinarians in order to have a script refilled. A member of our team will contact you to advise if a revisit is required before medications can be dispensed.
